Registration Requirements: Visit us between 8:30 A.M. and 3:00 P.M. to fill out a Pre - Intake application for your child to be placed on our “Wait List”.
You should also bring;
Child's birth Certificate, Proof of Income,Proof of Child's Immunizations, 2 proofs of Address, Child's Medical Insurance Card,Parents ID.

Child’s completed medical form, Child’s completed dental form, List of authorized adults.
